Healthy Howard is guided by the prin­ci­ple that being healthy requires the par­tic­i­pa­tion of both pub­lic and pri­vate enti­ties and orga­ni­za­tions that are an inte­gral part of an individual’s life and together com­pose their community.The pro­grams of Healthy Howard focus on improv­ing access to health care, pro­mot­ing healthy lifestyles by build­ing strong com­mu­nity part­ner­ships for health and pro­vid­ing care coordination.

Com­mu­nity Part­ner­ship for Health

Healthy Howard works with a vari­ety of local groups and busi­ness to help con­nect peo­ple like you to hav­ing a health­ier and hap­pier life:

Jour­ney to Bet­ter Health: Faith-Based Health Ini­tia­tive works with faith-based orga­ni­za­tions to help improve the health of their over­all congregants.

HEAL Zone (The Healthy Eat­ing and Active Liv­ing Zone) is an inno­v­a­tive effort work­ing in the North Lau­rel, Sav­age, and Jes­sup com­mu­ni­ties to bring resources to an area known for high health disparities.

Local Health Improve­ment Coali­tion (LHIC): Man­aged by Healthy Howard, LHIC is a group of over 40 orga­ni­za­tions focused on address­ing health dis­par­i­ties and improv­ing well­ness and pro­mot­ing health in our community.

Care Coor­di­na­tion

Healthy Howard offers a vari­ety of resources that can help find you a reli­able and inex­pen­sive health plan.

Com­mu­nity Care Team helps chron­i­cally ill indi­vid­u­als con­nect with resources that help them to bet­ter man­age their ill­ness, improve their health, and avoid pre­ventable hospitalizations.

Advanced Pri­mary Care pro­gram works to improve pri­mary care deliv­ery for Howard County res­i­dents by help­ing providers work toward Patient Cen­tered Med­ical Home(PCMH) standards.

Access to Care

Door to Health­Care was cre­ated to bring effi­cient access to West­ern Howard County res­i­dents who are nav­i­gat­ing their way through pub­lic health care options. Clients can speak to a bilin­gual staff mem­ber over the phone or in-person about their eli­gi­bil­ity for health­care pro­grams, find out the sta­tus of their cur­rent appli­ca­tion, and get con­nected to health­care providers that accept their insur­ance plan.

Re-entry Health Ser­vices pro­gram helps indi­vid­u­als who are released from the Howard County Depart­ment of Cor­rec­tions facil­i­ties to have health cov­er­age and are con­nected with the com­mu­nity resources needed to access care. Access to care has been shown to decrease recidi­vism rates and con­tribute to health­ier com­mu­ni­ties. As of July, 2016, this pro­gram is admin­is­tered by the Howard County Depart­ment of Corrections.

Healthy Lifestyle

Healthy Howard, Inc. now runs a num­ber of vol­un­tary com­mu­nity health pro­mo­tion pro­grams focused on key insti­tu­tions, these include:

Health­i­est Mary­land Busi­nesses is a statewide move­ment to cre­ate a cul­ture of well­ness at work.

Healthy Restau­rants pro­gram encour­ages restau­rants to offer healthy foods to con­sumers din­ing out in Howard County. There are more than 120 par­tic­i­pat­ing restau­rants that offer con­ve­nient, acces­si­ble and deli­cious dining.

Healthy Schools is com­mit­ted to improv­ing the health and well-being of Howard County stu­dents and their families.
