Journey to Better Health: Faith-Based Health Initiative works with faith-based organizations to help improve the health of their overall congregants.
HEAL Zone (The Healthy Eating and Active Living Zone) is an innovative effort working in the North Laurel, Savage, and Jessup communities to bring resources to an area known for high health disparities.
Local Health Improvement Coalition (LHIC): Managed by Healthy Howard, LHIC is a group of over 40 organizations focused on addressing health disparities and improving wellness and promoting health in our community.
Healthy Howard offers a variety of resources that can help find you a reliable and inexpensive health plan.
Community Care Team helps chronically ill individuals connect with resources that help them to better manage their illness, improve their health, and avoid preventable hospitalizations.
Advanced Primary Care program works to improve primary care delivery for Howard County residents by helping providers work toward Patient Centered Medical Home(PCMH) standards.
Access to Care
Door to HealthCare was created to bring efficient access to Western Howard County residents who are navigating their way through public health care options. Clients can speak to a bilingual staff member over the phone or in-person about their eligibility for healthcare programs, find out the status of their current application, and get connected to healthcare providers that accept their insurance plan.
Re-entry Health Services program helps individuals who are released from the Howard County Department of Corrections facilities to have health coverage and are connected with the community resources needed to access care. Access to care has been shown to decrease recidivism rates and contribute to healthier communities. As of July, 2016, this program is administered by the Howard County Department of Corrections.

Healthy Restaurants program encourages restaurants to offer healthy foods to consumers dining out in Howard County. There are more than 120 participating restaurants that offer convenient, accessible and delicious dining.
Healthy Schools is committed to improving the health and well-being of Howard County students and their families.
